Certificate Collection

- Marriage certificate or letter from the Customary Court/Dept. of Civil Registration for surname changes.
- Authorization affidavit if collected by someone other than the certificate owner.
Certificate Replacement
- Required Documents:
- The affidavit states the exam year, school name, and reason for the replacement.
- Identity document.
- P150.00 fee if within 18 months; P250.00 if after 18 months.
- Affidavit for name/spelling discrepancies.
- Marriage certificate or official letter for surname changes.
- Authorization affidavit if collected by someone other than the owner.
Certificate Correction
- Correction Period: Corrections are allowed within 18 months of issuance.
