• Required Documents:
  • Identity document (OMANG for citizens; passport for non-citizens).
  • P250.00 fee for certificates uncollected after an 18-month grace period.
  • Affidavit for name differences or spelling errors.
  • Marriage certificate or letter from the Customary Court/Dept. of Civil Registration for surname changes.
  • Authorization affidavit if collected by someone other than the certificate owner.

2. Certificate Replacement

• Required Documents:
  • The affidavit states the exam year, school name, and reason for the replacement.
  • Identity document.
  • P150.00 fee if within 18 months; P250.00 if after 18 months.
  • Affidavit for name/spelling discrepancies.
  • Marriage certificate or official letter for surname changes.
  • Authorization affidavit if collected by someone other than the owner.

3. Certificate Correction

• Correction Period: Corrections are allowed within 18 months of issuance.
• Required Documents:
  • Letter from the school for amendments or affidavit for private candidates.
  • Original certificate and certified ID copy.
  • Only spelling and date of birth corrections are allowed; no additions or surname changes.

4. Certificate Merging (JCE Only)

• Required Documents:
  • Letter requesting the merge with the years and schools listed.
  • Identity document.
  • Fee: P600.00 (for merging certificates within the past five years).
  • Affidavit for name/spelling discrepancies.
  • Merging is allowed only once per certificate.

5. Enquiries About Results

• Required Documents:
  • Request a letter from the school or center.
  • Fee: P250 per component for a review; no fee for “X” inquiries.
• Submission: Within 6 weeks of results release; private candidates submit directly, while school candidates submit through their center.
